I need help with aligning the heater flap actuator

This is the small servo motor the opens and closes the heater flaps. My struggle is when I removed the heater box and A/C, I removed the actuator as it was loose and missing one of the mounting screws. Shortly after that I had to undergo shoulder surgery and was unable to work on the car for about 8 months. It's now about two years later and I don't know how to properly orient the motor and lever (p/n 64111380502)  to the flaps. 

I've tried to find pictures here and everywhere that shows the assembly but have been unsuccessful. Anyone know how to properly orient the actuator? With flaps fully closed? open? Better yet, do you have a picture?
